Athletic Physical Exam Required



Each athlete participating in an OHS sport must have a sports physical.

Athletes must submit an examination card that has been completed and signed by the athlete, parent and a doctor.

Student-athletes have the option of attending a group health exam arranged by the school or of having the check-up performed by his personal physician.

If the athlete chooses to have his doctor perform the exam, the student must take the card to clinician's office to be completed. Physical-cards are available at the OHS Athletic Office or Main Office.

The school arranges group sports physicals, once per year, that are included in the cost of the annual sports registration fee.

Sports physicals are conducted by the school on designated dates in the spring of each school year.

Physicals must be completed and dated no earlier than April 15, 2010 to be valid for the 2010-2011 school year. Physical-exams must be completed and cards must be submitted to the coach prior to the beginning of the season.



Please make a copy of both sides of the completed physical-exam card.

There are hundreds of athletes participating in OHS sports in any given season. That means hundreds of forms for the athletic office to manage. There have been a few instances of physical-cards being misplaced. Keeping a copy of your form is a good way to avoid any problems with last minute ineligibility due to a missing card.
