Emergency Contact, Code of Conduct & Athletic Eligibility On-line Form

Before beginning team practices or try-outs, athletic participants must have a completed an Emergency Contact Form, a Code of Conduct Form, as well as an Athletic Eligibility Form. This combined form is available for completion on-line through the Okemos High School Athletic Office Website

Please make sure that you have completed these electronic forms before try-outs begin for the fall 2008 season.
